Dazzling Pamper Parties will either need to have completed parent/guardian consent forms for each child attending a party (under the age of 16), or verbal consent at the party from the host parents on behalf of the guest children's parents.

All personal information is completely confidential and for the sole use of Dazzling Pamper Parties and will not be disclosed to any third parties. Dazzling Pamper Parties reserve the right to refuse treatments to any child whose parent/ guardian has not signed the aforementioned consent form or given verbal consent to either Dazzling pamper parties or the host parent of the party.
